1.How would you describe your periods the last 12 months? If you have not had any, please select “I have not had any periods in the last 12 months”
2.If you didn’t have a period in the last 12 months, did your period stop because of surgery to remove both of your ovaries? (If you have had a period in the last 12 months, select “I have had a period in the last 12 months.”)
3.If you didn’t have a period in the last 12 months, did your period stop because of chemotherapy or radiation? (If you have had a period in the last 12 months, select “I have had a period in the last 12 months.”)
